WAIVER, RELEASE, AND ASSUMPTION OF RISK
By submitting this form, I acknowledge and agree to the following:
1. Assumption of Risk:
I hereby assume all risks of participating in this activity or event organized by RTS Tactical, including, but not limited to, risks that may arise from negligence or carelessness on the part of RTS Tactical, its employees, volunteers, or agents; risks from dangerous or defective equipment or property; and any other risks inherent in the activity.
2. Waiver and Release of Liability:
I waive, release, and discharge RTS Tactical, its directors, officers, employees, volunteers, representatives, and agents from any and all liability, including but not limited to liability arising from negligence or fault, for my death, disability, personal injury, property damage, property theft, or actions of any kind that may occur to me during or as a result of my participation in this activity or event, including travel to and from the event.
3. Media Release:
I grant RTS Tactical the irrevocable right and permission to capture my image, likeness, and/or voice in photographs, videos, or other media ("Media") during my participation in this activity or event. I further grant RTS Tactical the right to use, reproduce, publish, and distribute the Media in any and all media now known or hereafter developed, including but not limited to social media, websites, advertising, and promotional materials, without compensation or further consent from me.
4. Certification of Eligibility:
I certify that I am 18 years of age or older, have never been convicted of a felony, and have never been charged with any form of domestic violence as defined by Florida Statute. I further certify that I am legally able to carry a concealed weapon in this state, if applicable to the activity.
5. Electronic Consent:
By submitting this form, I acknowledge that I have read and fully understand the terms of this Waiver, Release, and Assumption of Risk. I am submitting this form voluntarily and of my own free will, and this submission serves as my electronic signature and agreement to these terms.
